- 2 Regular Cleaning or Prophylaxis
- 2 Exams and X-rays
- 1 Fluoride treatment
- 20% off office fees on treatments
Membership Advantages
- No deductible
- No maximum allowable
- No waiting period
- No missing tooth clause
- No frequency exclusions
PROVISIONS:
- Membership fee is due at time of enrollment
- Plan duration is one (1) year from sign-up date
- Payment in full must be paid on date of service
- Cannot be combined with any other insurance or savings plan
- Unused services within the plan duration do not carry over
- Fees are based on office fees, not insurance fees
EXCLUSIONS & LIMITATIONS:
- Not transferable to another party
- Membership fee is not refundable after 30 days, or if any benefits have been used
- The benefits of this plan are limited to procedures completed only at Allure Dental locations
- The benefits of the plan can not be applied to treatment already in progress
- Discount is excluded when the cost of treatment will be covered by workmen’s compensation, or any government entity or business due to injury, etc.
A $50 fee will be charged for missed appointments if we are not notified within 24 hours.
